Group Tours Request Form

Please download the PDF and fill it out to request a group tour. You can email the PDF to gshmuseum@hotmail.com or drop it off at the museum. Once the request form is processed someone from the museum will contact you to finalize the tour. If you have any questions please call us at 712.246.1669.
